Type-2 Diabetes & Metabolic Surgery in Lucknow

Type-2 diabetes and obesity are closely linked, and for selected patients whose diabetes is driven by excess weight, metabolic surgery — the use of weight-loss operations to treat metabolic disease — can produce remarkable improvement, and sometimes remission, of diabetes. This is not a treatment for everyone with diabetes, but for the right patient it can be life-changing.

Dr Sandeep Kumar Verma assesses patients in Lucknow for metabolic surgery at Medanta, working alongside physicians and dietitians to determine who is likely to benefit.

What is metabolic surgery?

Metabolic surgery refers to operations — chiefly the gastric bypass and sleeve gastrectomy — used specifically to improve type-2 diabetes and related conditions in people with obesity. Beyond weight loss, these operations change gut hormones in ways that improve the body's handling of blood sugar, often with benefits seen within weeks, before much weight is lost.

It is important to be clear: this is an option for selected patients with obesity-related type-2 diabetes, decided together with your diabetes physician — not a replacement for good diabetes care in general.

Symptoms to watch for

  • Poorly controlled blood sugar despite medication, in the context of obesity
  • Need for increasing doses or numbers of diabetes medicines
  • Coexisting high blood pressure, high cholesterol or fatty liver
  • Obstructive sleep apnoea and reduced quality of life

Causes & risk factors

  • Obesity and insulin resistance
  • Genetic and lifestyle factors
  • Often part of a wider 'metabolic syndrome'

How it is diagnosed

  • Assessment of BMI, diabetes control (HbA1c) and duration
  • Review of current medication and other obesity-related conditions
  • A full bariatric and metabolic work-up before any surgery

How metabolic surgery helps diabetes

In suitable patients with obesity-related type-2 diabetes — generally with a BMI of 32.5 or above using Asian-Indian thresholds — a gastric bypass or sleeve gastrectomy can lead to major improvement or remission of diabetes, often allowing medication to be reduced or stopped. The gastric bypass in particular has a strong effect on blood sugar.

Results are best when diabetes has not been present for too many years and when surgery is combined with ongoing medical care. The decision is always made jointly with your physician, and surgery is one part of a complete metabolic-health plan.

When to see a GI surgeon

  • Obesity with type-2 diabetes that is hard to control on medication
  • Rising medication needs and other obesity-related conditions
  • A wish to explore whether metabolic surgery is an option for you
Frequently Asked Questions

Type-2 Diabetes (Metabolic Surgery): common questions

Can surgery reverse type-2 diabetes?

For many patients with obesity-related type-2 diabetes, metabolic surgery leads to remission or major improvement, often within weeks, and sometimes allows medications to be reduced or stopped. It works best when diabetes has not been present for many years. It is an option for selected patients, decided together with your diabetes physician — not a cure for all diabetes.

Who is a candidate for metabolic surgery?

It is generally considered for adults with obesity-related type-2 diabetes and a BMI of 32.5 or above (Asian-Indian threshold), particularly when diabetes is hard to control. A full assessment with the surgical and medical team determines whether it is likely to benefit you.
